Request Test Scores

You may view, print, and request that your placement test scores be shared with other institutions.

You may request your placement test scores be sent to your employer by mail, email or in person.

Request by Mail

Mail your completed Scores Request form to:

Attach your completed Scores Request form to an email to [email protected] or [email protected].

Request in Person

You may deliver your completed Scores Request form to Testing Center on our East Peoria Campus, Administration Building, L220 or Peoria Campus, Arbor Hall, 103. You must bring your completed request form and a valid picture ID to have your scores sent.
