Joint Membership

I/We herby agree to accept and abide by the terms of this Membership Application and Agreement. I/We understand that this membership agreement will continue on a month-to-month basis unless cancelled by me.

I/We (Member) herby authorize ICC Fitness Center to withdrawal payment fro monthly dues and approved membership charges for the duration of my/our membership through electronic funds transfer on or about the 20th of each month. This authorization is to remain in full effect until ICC Fitness Center has received WRITTEN NOTIFICATION, fax, or email from me (either of us) on CANCELLATION, on or before the 15th of the month.
